Consumer Claims Information
In the event of a breakdown, follow these claim procedures.

For emergency repairs follow the claims procedures without the authorization number. However, you MUST contact Prizm within five (5) business days of the repair date to determine if the repair is covered under the service agreement.

  • Prevent further damage by taking immediate action. Continuing to operate or not securing a timely repair negates the service agreement's coverage.
  • Take your vehicle to a licensed repair facility.
  • Provide the repair facility with a copy of your service agreement or your service agreement number.
  • Obtain authorization from Prizm Administrative Solutions, Inc. (Prizm) at 800-356-6459.
  • Authorize tear-down or inspection.
  • Review coverage with the service manager.
  • Pay any applicable deductible. Reimbursement will be for repairs based on the service agreement and authorization, less the deductible. Documentation for reimbursement must be made within thirty (30) days to be eligible for payment. If you cannot take your vehicle back to the selling dealership or to an alternate repair facility that will accept the authorization number, we can make payment using a credit card.
